Out of Network Physical Therapy
Empowered SF Physical Therapy is an out-of-network provider for all insurance plans. This allows for one-on-one, individualized care focused on your specific goals without many of the restrictions that can come with insurance-based care, such as shortened visits, high patient volume, or limitations on treatment approaches.
Our goal is to provide thorough evaluations, movement based treatment, education, and individualized exercise programs to help you become independent in managing your symptoms and returning to the activities important to you.
Payment is due at the time of service. A superbill will be provided for possible reimbursement through your out-of-network benefits. HSA and FSA cards are accepted.
Medicare Information
Original Medicare:
For patients with Original Medicare, we are a non-participating Medicare provider. This means payment is collected at the time of service and claims are submitted by us directly to Medicare on your behalf.
Medicare typically reimburses 80% of the approved amount. Medicare then sends reimbursement directly to you, typically by mailed check or direct deposit depending on your Medicare setup. If you have a Medicare supplemental (Medigap) plan, it may cover some or all of the remaining balance depending on your specific plan.
For many patients with Original Medicare and a supplemental plan, the overall reimbursement is often similar to what they would receive with a participating Medicare provider. The primary difference is that payment is made upfront at the time of service.
Medicare Advantage Plans:
Please note that we do not participate with Medicare Advantage plans.
Some Medicare Advantage plans may offer limited out-of-network benefits; however, coverage and reimbursement vary widely by plan. Patients are encouraged to contact their insurance provider directly to determine whether out-of-network physical therapy services are covered under their specific policy.

FAQ
Do I need a physician’s referral?
For most patients, a physician referral is not required to begin physical therapy in California.
However, patients using Original Medicare are required to have a referral or prescription from their physician prior to starting treatment.
Patients with Medicare Advantage plans should check with their insurance provider regarding referral and authorization requirements, as these vary by plan.
If you are unsure whether you need a referral, feel free to contact the clinic prior to scheduling.
Can my insurance reimburse me for out-of-network visits?
Yes, many insurance plans offer out-of-network reimbursement. While we don’t bill insurance directly, we provide a detailed superbill that you can submit to your insurance provider for possible reimbursement. Coverage varies by plan, so we recommend contacting your insurance company to ask about out-of-network physical therapy benefits, deductibles, and any required documentation. We’re happy to walk you through the process or answer any questions about using a superbill.
Do you accept health spending accounts and flexible spending accounts?
Yes! Physical therapy is typically an eligible expense under most Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA). You can use your HSA or FSA card to pay directly at the time of service. If needed, we’re happy to provide a detailed receipt or superbill for your records or reimbursement purposes. If you're unsure about your account’s coverage, we recommend checking with your HSA/FSA provider for specific eligibility.
Do you have a cancellation policy?
We kindly ask that you provide at least 24 hours’ notice if you need to cancel or reschedule your appointment. Appointments canceled with less than 24 hours’ notice — or missed entirely — will be charged a $75 cancellation fee. All cancellation fees must be paid in full before booking your next appointment. We appreciate your understanding as this policy allows us to offer the best care to all our clients.
